This is variant of my Garry's mod hand.

This requires http://www.garrysmod.org/downloads/?a=view&id=123802 (the my gmod hand)
Just extract that(my gmod hand) to <STEAMDIR>/steamapps/<YOURACCOUNT>/garrysmod/garrysmod
then extract this(this variant) to <STEAMDIR>/steamapps/<YOURACCOUNT>/garrysmod/garrysmod
or it won't look good and doesn't have a self illuminating.
